  • Alton Memorial claims Bottoms received medical care at the hospital beginning on Oct. 12, 2004, which has not been paid. Alton Memorial is seeking $129,693.19 in damages.

  • Mack claims that on June 13, 2004, she was permanently injured when Williams allegedly collided with her on Route 111 in Wood River. Mack is seeking at least $50,000 in damages.

    Linda Harvell, Samantha Harvell v. Cameron Mank

  • The Harvells claim they were seriously and permanently injured on June 27, 2003 when Mank allegedly collided with them on Horseshoe Lake Road and Eastport in Collinsville. The Harvells are seeking at least $50,000 in damages.

  • Westerhold claims Michael Rallo, M.D. failed to order an electrocardiogram and stress test on Feb. 16, 2004, after her husband reported symptoms of chest pain, dizziness, and shortness of breath. Eric Westerhold died July 10, 2004, of coronary artery disease. Westerhold is seeking damages in excess of $50,000.
