BELLEVILLE — A nursing and rehab facility located in Lebanon is facing a claim from a former patient's estate alleging negligent care led to the patient's fall, pressure ulcers and infection.

Tre'vien Teer, as independent administrator of the Estate of Tequlia Anderson, deceased, filed a complaint July 28 in the St. Clair County Circuit Court against Cedar Ridge Care and Rehabilitation Center LLC, doing business as Cedar Ridge Health and Rehab Center, and Covenant Care California LLC, alleging violation of the Illinois Nursing Home Care Act.

According to Teer's complaint, Anderson was a resident of Cedar Ridge Health and Rehab Center located on Perryman Street in Lebanon from July 19, 2018, through April 5, 2020. He claims Anderson suffered a fall on Nov. 1, 2019, after being left alone in her room in the middle of being changed and hit her head on the dresser, requiring hospitalization. The plaintiff further alleges that after returning to Cedar Ridge, Anderson suffered from pressure sores on her arm and hand and suffered sepsis and a sacral pressure ulcer.

The plaintiff claims the defendants failed to provide adequate medical and personal care to Anderson and failed to provide supervision and assistance with daily living, including dressing, eating, ambulating, bed transfers and other care.

Teer seeks monetary and all other just relief.
