A Smithon nursing home is accused of refusing to provide a resident’s daughter with his medical records following his death.
Ambrea Douglas, natural daughter of Jack Douglas, filed the complaint Sept. 17 in the St. Clair County Circuit Court against Park Haven Nursing & Rehabilitation Center LLC, doing business as Integrity Healthcare of Smithton.
According to the complaint, Jack Douglas was a resident at Integrity Healthcare located on South Lincoln in Smithton. He was discharged to Belleville Memorial Hospital on June 4 and died days later on June 8, the suit states. Following his death, attorney Stephen Buser of Columbia, Ill., sent a letter to Integrity Healthcare requesting copies of the decedent’s records on behalf of his daughter.
The suit states that as of Sept. 16, Integrity Healthcare has refused to provide the records. Ambrea Douglas claims she has been prejudiced by the defendant’s refusal to provide documents as she has incurred unnecessary attorneys fees and legal expenses.
She seeks an order requiring the defendant to provide the complete chart and records of her father, plus attorneys fees and all other relief the court deems just.
